Position Description

The RN delivers clinically competent care to patients utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. RNs will effectively interact with patients, significant others, and other health team members while maintaining all standards of professional nursing.

 Essential Duties and Responsibilities

1.       Use the nursing process by demonstrating appropriate assessment, planning, implementation and evaluation of the patient's care, as evidenced by guest/staff observation.

·      Assess the patient: collect and document the patient's current health status and medical history. Complete and/or review Admission Data Record as defined by policy. Make accurate and ongoing assessment of patient's status and respond appropriately.

·      Plan/implement: set priorities for nursing action in a logical sequence according to patient's needs and formulate a plan of care by the appropriate selection of Nurse Practice Standards. The plan of care is initiated on patient admission based on all aspects of the initial assessment. Plan includes physiological, psychosocial and environmental factors: patient/significant other education and patient discharge planning.

·      Evaluate: perform ongoing assessment and revise plan of care based on new data and patient's condition.

·      Document accurate and ongoing assessment of patient status with Sandia Pueblo guidelines: Document patient care, including nursing intervention, patient response to care provided, patient needs, problems, capabilities, limitations, and progress toward goals. Documentation includes evidence of appropriate patient/significant other teaching, and the understanding of these instructions is noted in the medical record.

2.       Communicate with the physician regarding patient's needs, the nursing assessments and recommendations daily and as needed, as evidenced by client feedback and operations coordinator observation.

3.       Collaborate with other nurses and health Team Members to ensure the patient's wellbeing as evidenced by staff/client feedback.

4.       Teach the patient/partner appropriate health information in a timely manner and share written informational material as evidenced by client feedback and operations coordinator observation.

5.       Perform direct patient care, using established procedures, policies and standards as evidenced by client/staff feedback and operations coordinator observation.

6.       Administer medications and IVs according to policy and procedure.

7.       Observe and document patient's response to pertinent medications.

8.       Demonstrate ability and appropriate technical skills when administering treatments and procedures in accordance with physician's orders and policy and procedure, within a timely manner.

 

9.       Demonstrate necessary skills and knowledge to provide care for patients according to policy, procedure and division/unit specific competencies.

10.   Demonstrate ability to handle emergency situations in a prompt, precise and professional manner.

11.   Provide personal patient care to ensure comfort and well-being to the patient, acknowledging physiological and psychological needs.

12.   Perform indirect patient care, using established procedures, policies and standards as evidenced by client/staff feedback and operations coordinator observation.

13.   Give a thorough, concise report at change of shift. Communicate pertinent information to the appropriate person in a timely manner.

14.   Demonstrate accurate and timely documentation according to form guidelines.

15.   Accurately transcribe physician orders according to policy and procedure.

16.   Maintain a clean, neat and safe environment for patients and staff according to policy and procedure as evidenced by staff/client feedback and operations coordinator observation.

17.   Ensure that supplies and equipment necessary for patient care are stored in an organized and efficient manner.

18.   Demonstrate correct and safe techniques in the use of equipment according to specific product information.

19.   Follow appropriate procedure for obtaining and returning or cleaning/disposing of equipment and supplies.

20.   Participate in economical utilization of supplies and ensure that equipment is maintained in a clean and safe manner.

21.   Adhere to administrative system/nursing service policies as evidenced by operations coordinator.

22.   Remain conscientious regarding personal hygiene and comply with specific division/unit regulations.

23.   Comply with specific system/division/unit policy and procedure concerning safety.

24.   100% attendance to mandatory skill updates yearly required.

25.   Attend staff meetings and any other relevant meetings.

26.   Serves as a coach and mentor for other positions in the department.

27.   Punctual and regular attendance is an essential responsibility for all team members and are expected to report to work based upon the assigned schedule each day in accordance with the Attendance Policy for Pueblo of Sandia.  

 Additional Responsibilities

1.       Demonstrates strong interpersonal skills in building a cohesive working relationship with co-workers and internal/external contact.

2.       Performs other job-related duties as assigned.

This position has Key Performance Indicators (KPI’s) identified as a measurement of success.  KPI’s will be shared with all team members who work in this job title and feedback will be provided regarding the successful completion of the KPI’s as part of the performance review process.

13.   Ability to perform physical requirements of the position (see ‘Working Conditions’).

14.   Ability to render basic First Aid and CPR.

15.   Ability to learn new skills and information as needed.

16.   Knowledge of tribal health concerns and available community resources.

17.   Knowledge of PSHC policies and procedures.

18.   Knowledge of safety regulations, standards and practices.

19.   Skill in relating to patients and other staff.

20.   Ability to recognize abnormal symptoms and/or identify problems in adults and children and report them to a provider on behalf of the patient as soon as possible.

21.   Ability to advocate for the patient during medical and non-medical appointments.

22.   Ability to demonstrate dependability, i.e. compliance with established work schedules.

23.   Ability to work a flexible schedule with days, evenings, nights, weekends and holidays as needed.

24.   Ability to read and write adequately to complete required documentation and interpret all written communication from PSHC supervisors or administrators.

25.   Knowledge of the community or population to be served preferable or extensive multi-cultural experience.

   Education and Experience  

Required:

1.       High School Diploma, GED certification or equivalent.

2.       Associate degree in nursing from an accredited college or university and four (4) years of experience in Outpatient primary care clinic management and two (2) years in health care management.

3.       Four (4) years of experience providing care in Outpatient primary care clinic setting and two (2) years in health care maintenance.              

Note:  Relevant work experience or education may be substituted to satisfy education and/or work experience. 

License/Certifications/Registrations

1.       Current New Mexico RN Licensure with the NM Board of Nursing.

2.       Current CPR and First Aid certifications

3.       The following immunizations and titers are required prior to employment start date:

·      Hepatitis B series or immune by titer

·      Tdap

·      MMR or immune by titer

·      Annual TB test

·      Annual Flu vaccination (seasonal)

·      COVID booster (optional but encouraged)

4.       Must be able to successfully pass a stringent background investigation and character investigation in compliance with PL 101.630.

5.       Will require a pre-employment and random drug screening.
